This is the 18-12 bias flap, 11.75" wide with a center valve hole for 18" rims. It fits 44x16-18 tires.
Fit the flap over the tube, run the valve stem through the center valve hole and mount the assembly so the flap lies flat against the rim base. Pair it with a Premium Service inner tube in the same size for a complete tube-type setup.
