This is the 16-10 bias flap, 11.5" wide with a center valve hole for 16" rims. It fits 40x14-16 tires.
Fit the flap over the tube, run the valve stem through the center valve hole and mount the assembly so the flap lies flat against the rim base. Pair it with a Premium Service inner tube in the same size for a complete tube-type setup.
